Hospital and skilled nursing facility social workers, along with SUD and mental-health programs, can connect members experiencing homelessness or unstable housing with medical respite, also called recuperative care. Eligibility, authorization, and placement are plan-dependent.

The member must be experiencing homelessness or unstable housing and meet the relevant plan requirements. Empowering Potential Housing currently contracts with Molina and Kaiser; eligibility, authorization, and placement are plan-dependent. Acceptance of other plans is not promised.
