How to qualify for Recuperative Care:

 

At Empowering Potential Housing (EPH) we want to support our community and end the suffering of homelessness.  We have a heart for those recovering from the horrible disease of addiction along with mental health issues that often accompany SUD (substance use disorder).  

 

If you'd like to see how EPH can provide Recuperative Care, please follow the below steps:

 

1. Ensure that you are active and current with Molina Medi-Cal.  At the present moment we are only contracted with Molina Healthcare.  It is entirely voluntary to change to Molina.   If you would like to change to Molina, call the Medi-Cal Managed Care Ombudsman 888-452-8609 - they are available Monday through Friday, 8 a.m. to 5 p.m. PST, excluding holidays.

 

 

The Ombudsman's office can also help you qualify for Medi-Cal through Molina if you are not currently covered along with assisting you move your Medi-cal plan from another County.

 

 

2.  Get your Doctor(s) or Emergency Department(s) to FAX to 619-333-1670 medical documentation that would prove the reason for needing recuperative care.  Here are some examples authorization letters from Doctors:

 

  • Client has just been released from prison _________ (date) and is homeless (code Z59.00=Homeless Unspecified).  Client has a chronic condition of SUD drug of choice = alcohol (Dr. would list the CPT code) along with PSTD (CPT code).   Client would run the risk of relapse if they stay in a shelter and must have the medical daily monitoring offered in Recuperative Care.  Client also has the following serious health conditions:  COPD (CPT code), high cholesterol  (CPT code).  Client needs Recuperative care to heal and rest from chronic health condition along with engaging in treatment providers now that Client is no longer incarcerated.  



  • Client has just been released from SUD inpatient facility __________ (date) and is homeless (code Z59.00=Homeless Unspecified).  Client has a chronic condition of SUD drug of choice = Meth (Dr. would list the CPT code) along with Bipolar Disorder (CPT code).   Client would run the risk of relapse if in a shelter and needs the daily medical monitoring offered in Recuperative Care.  Client also has the following serious health conditions:   diabetes (CPT code), cardiovascular disease (CPT code).  Client needs Recuperative Care to heal and rest from their chronic SUD condition along with engaging in treatment providers now that Client is no longer in-patient.



  • Client has just been released from SUD Recovery Residence funding __________ (date) and is homeless (code Z59.00=Homeless Unspecified).  Client has a chronic condition of SUD with drug of choice = Meth (Dr. would list the CPT code) along with Anxiety Disorder (CPT code).   Client would run the risk of relapse if in a shelter and needs the medical daily monitoring offered in Recuperative Care.  Client also has the following serious health conditions: Diabetes  (CPT code) asthma  (CPT code).  Client needs Recuperative care to heal and rest from their chronic SUD condition and avoid relapse and visits to the Emergency Department.  Prior to engaging with IOP program, client frequented the ED on the following dates:  _________, ________________, _____________ (list all dates within the last 12 months).



3.  Book a Google meeting with Rachel.  Click here to book the meeting/call.   

Book this meeting only AFTER your Doctor(s) have faxed letters to 619-333-1670.

 

Be ready to answer these questions during the meeting - Name, date of birth, Molina Medi-Cal ID number, where have you been living up until now, any sources of income, what are your goals for housing?

 

If you are a referring entity like a SUD IOP, SUD in-patient treatment, etc. you can complete the Recuperative Care Referral form and fax to us directly at 619-333-1670 with Empowering Potential Housing listed as Requested Provider in the first section along with sending the required medical provider documentation.

 

Please note - we have NO CONTROL over approvals with Molina and you will need to self-pay for the nights until you are qualified ..... or plan in advance before your funding source/Recovery Residence funding ends.
